Develop individualised treatment and follow-up plans for patients with cardiovascular conditions.
Manage long-term care for patients with chronic heart disease and related cardiovascular conditions.
Educate patients on cardiovascular health, disease prevention, lifestyle modification and treatment adherence.
Coordinate with multidisciplinary healthcare teams in routine patient care and cardiovascular emergencies.
