Home- and Community-Based Care Worker provides personal care, support with daily living tasks and companionship to individuals who need assistance due to ageing, illness, disability or recovery from health setbacks in private homes or community settings. Workers help clients maintain independence by supporting hygiene, mobility, meal preparation, medication reminders, light housekeeping and emotional wellbeing, while liaising with families and healthcare professionals as needed.
Tasks
Assisting individuals with personal hygiene, bathing, grooming, dressing and toileting.
Helping with feeding, meal preparation, nutritional support and medication reminders according to care plans.
Supporting mobility including safe transfers, walking assistance or wheelchair use.
Performing light housekeeping tasks such as laundry, cleaning and changing bed linen.
Monitoring individual condition, recording changes and reporting concerns to supervisors or health professionals.
Providing companionship, emotional support and encouragement for daily activities.
Assisting families with caregiving strategies and community resources.
